- Mail It—don’t hand deliver. This is best for your family.
- Send It to the Superintendent’s Office—no other school office is specified by the law.
- Use the Following Form—not the one provided by the school district. Many districts seek information not required by law. This form meets all requirements … and nothing more. (approved by HSLDA)
